Calendar Request Form

Use the above form to request that an event be added to the official Church Calendar.  Fill in the form electronically and email it to the leader of the ministry sponsoring the event.  If the leader of that ministry approves the event, have them email the form to pastordoug@pathwayofhope.net for final approval.

Event Planning Checklist

Use the above form to assist in planning an event.  The person responsible for the event should fill in the form electronically and email it to pastordoug@pathwayofhope.net at least two weeks prior to the event for final approval (this gives enough time to make any changes or last-minute arrangements if necessary).

Connection Group Covenant

The above document is to be signed by all members of a Connection Group and given to Pastor Doug, who will make a photocopy and return it to the Connection Group Leader.  Please email Pastor Doug at pastordoug@pathwayofhope.net with any questions.
